Resection of the lower jaw

The incision is made in the oral cavity from the second molar to the canine. The desired shape is cut along the line from the back of the jaw to the chin opening. How is the procedure performed?

T-osteotomy

The incision is made inside the mouth from the canine to the canine, below the gum line. One horizontal and two vertical incisions are made to the jaw bones. Another horizontal slice can be made to further reduce the length of the chin. The central bone fragment is removed, and the rest of the bone is fixed with a titanium plate and screws. Soluble material is also available. Then the bottom line of the chin is ground to create a smooth line. This procedure is often used in resection of the lower jaw.

Direct resection (cut)

This procedure is suitable for people who do not need to correct the position of the chin, but want only to reduce the width of their chin and jaw. Also used to correct minor asymmetry of the chin.

How is the procedure performed?

Different approaches are used depending on the position and the line of the patient's teeth. The clinics use 3D CT, X-rays and virtual 3D surgery to determine the best approaches.

Traditional approach

For 1-2 years before the operation on the jaw, teeth are aligned in an optimal way through orthodontics. Then, after aligning the teeth, an operation can be performed. After, to achieve optimal alignment of the teeth, you can undergo minor orthodontic treatment.

Operation - First approach

This approach is used in cases where the position of the teeth is optimal for the operation. The total time for treatment is much shorter than for the traditional approach, because of the unnecessary preoperative orthodontic procedure. The jaw is corrected through surgery, and only then is orthodontic treatment necessary.

Modified operation - First approach

This approach combines a traditional approach and operation for cases where teeth have minor lineage problems or orientations. Preoperative orthodontic treatment can be done for a short period (3-4 months) and then proceed with a jaw surgery with postoperative orthodontic treatment.

Operation on the jaw

Upper jaw (LeFort I Osteotomy)

LeFort I osteotomy is the most commonly used procedure for cutting bones in the upper jaw. This procedure is performed under general anesthesia. The operation is performed completely in the oral cavity. A U-shaped incision is made on the gums of the upper teeth to gain access to the jawbone. Then the gum is lifted from the bone. The upper incisions of the jaw are made with the help of highly specialized instruments, which make it possible to clearly dissociate the tissues. If the upper jaw should be minimized, the additional bone is removed. Bone can be built if it is necessary to lower or lengthen the jaw (bone graft). The upper jawbone is then inserted into its final position (planned before your operation) and strengthened in place by tiny titanium plates and screws that remain in place permanently. Gums are sewn with soluble screeds, which dissolve within 3-4 weeks. After the bones heal, the jaw restores its strength.

Lower jaw

1) Sagittal Split Ramus Osteotomy (SSRO)

This procedure is used to correct the mandibular retraction and prognathism of the lower jaw (deep bite and jaw protrusion). First, a horizontal incision is made on the inner side of the lower jaw, expanding access to the front. Then, a cut is made downwards, spreads on the lateral border of the lower jaw in the area between the first and second molars. At the same time, a vertical incision is made, extending access to the jaw, to the lower border of the jaw. All incisions are made up to the middle of the bone where the bone marrow is located. The medical bit is then inserted into pre-existing incisions and gently punctured into all areas to divide the lower jaw into the left and right sides. From this position, the lower jaw can be moved forward or backward. If there is a shift in the opposite direction, the additional segment must be trimmed to make room to move the lower jaw in the opposite direction.

2) Varus Rotational Osteotomy (VRO)

Vertical osteotomy is a procedure in which the whole thickness of the branches of the lower jaw is cut. It is carried out on both sides, the lower jaw is divided into 2 small segments with condyles and onto a large segment consisting of the anterior part of the lower jaw and the part including the teeth and chin. The dissections are made behind the lower alveolar canals, and therefore the chance of damage to the alveolar nerve is minimized. This operation can be used for repositioning, changing the angle of the lower jaw, and also shortening the posterior part of the lower jaw. However, this operation can not be used to lengthen the lower jaw.

3) Anterior Segmental Osteotomy (ASO)

The upper and lower first molars are removed from both sides of the mouth along with the bone on which they are located. The front part of the upper and lower oral cavity is moved back and fixed to the jaw by means of titanium plates to firmly fix it. It is usually done for people with protruding mouth (bimaxillary protuberance).

Recessed chin

A cut is made of the oral cavity, the chin bone is cut and moved forward, and set using the technique of SSRO (Sagittal Split Ramus Osteotomy). Using the titanium plates, screws and / or wires, the jaw is fixed in place. The jaw must remain closed until the bones have healed.
